Continuity of Care Matters
We understand the trust and comfort that comes with long-standing healthcare relationships. That’s why Brentwood works to accommodate outside physicians, specialists, and therapists. Our nursing team collaborates with external providers to maintain up-to-date care plans, medication records, and treatment recommendations.
How We Coordinate Care
If a resident chooses to keep their doctor, our team will help coordinate communication, schedule transportation if needed, and ensure follow-ups are managed appropriately. We can also provide secure access to health updates upon request, making sure everyone stays on the same page.
On-Site Providers Are Also Available
For convenience, many families choose to work with our on-site physicians, nurse practitioners, and therapy partners. These providers are familiar with Brentwood’s care approach and are readily available in case of urgent needs or routine monitoring.
A Choice That Stays Personal
Whether a resident continues with their existing physician or transitions to on-site care, the decision is entirely up to the family and resident. Our role is to make that choice as seamless and supported as possible.
